A hiatal hernia occurs when the upper portion of the stomach ascends into the chest through a small opening that exists in the diaphragm (diaphragmatic hiatus). The diaphragm is the muscle that separates the thorax from the abdomen. This type of hernia is associated with gastroesophageal reflux in the majority of cases.
